The data produced from this sheet is only accessed by the Department of Health and Social Care's Information Team. While aggregate responses will be available to practices, it is suggested that you do not include information that makes identifying you easier. Individual responses will be held for up to 1 year, but then deleted.  Aggregate information in which individual responses are not identifiable will continue to be held.
